Elwyn Adult Behavioral Health Services seeks a Recovery Support Worker (RSW) to build relationships with individuals and develop shared understandings of the individuals' goals and the steps necessary to achieve the goals. ESSENTIAL FUNCTIONS: Reviews the Screening, Assessment, Crisis Plan, Critical Needs Plan and IAP upon completion or update completed by the LPHA. In conjunction with the Integrated Team/ GLE/ IGLE staff, assists persons with identifying and implementing meaningful goals for their Individual Action Plan focusing on the following areas: mental health recovery education, daily living skills, substance use, relationships, independent transportation, self-help groups, exercise/wellness groups and the Recovery Learning Center (RLC), vocational & leisure. Demonstrates knowledge of prescribed medications, ability to monitor medications, and follows FHR state specific medication guidelines.
